Important Instructions

• Treat this work as most important and submit the following at  Room No 312,Second Floor,Block-A, Mini sectt., Patiala.
  1. Excel Data sheet in CD/pendrive and write the name of department/office on CD with black marker. All Department Heads to ensure single CD/pendrive per Department.
  2. Declaration Certificate Signed and stamped by Head of the Department.
  3. Relevant Supporting documents (signed and stamped by Head of the Department) of the employees those are marked / remarks as Disable /Pregnancy /Lactating woman /On Long leave/BLO/medical certificate of very Chronic Patients etc. should also be submitted along with Declaration Certificate.
  4. Data of class 4 employees / drivers/security guards etc. should not be entered in Excel Data sheet.
•  Old data of your office/department is already provided in the Excel Data sheet file.
• Please download the excel sheet data file ,then find your department’s data and copy to new excel file .Use  MS excel and edit/add/ update it. 
• In case of transfer/ retirement please enter proper remarks in ‘Remarks’ column for modification the data.
• Do not DELETE the records. Please mention the reason in Remarks column i.e. ( Transferred/Retired etc )
• For newly joined employee in their office, enter all the details of employees in all the columns at the end of the list and enter proper remarks in ‘Remarks’ column i.e. New entry/ Joinee etc 
  Letter no. should be left blank.
•  If old data of your department/office is not available in this Excel Data sheet file then kindly enter all your employee’s data in Excel Data sheet file in given format.
• If there is ‘no change‘ in the old old data in Excel data sheet Kindly submit only the Declaration certificate hardcopy(Signed and Stamped) .
• Dise Capsule software installation is not needed for this work.
