Important Instructions
- Treat this work as most important and submit the following at Room No 312,Second Floor,Block-A, Mini sectt., Patiala before 10 September,2024.
- Exported data in CD and write the name of department/office on CD with black marker.
- Declaration Certificate by Head of the Department generated from DISE capsule (Reports —> Print Declaration Certificate along with Required documents.
- Summary Report of Records Entered (Reports —> Summary of Entered Records Available for Export)
- Signed Checklist from Individual Staff Member (Reports —> Checklist Signed by HOD).
- All pages of Checklist must be Signed and stamped by HOD.
- When data of all employees is entered in the DISE Capsule or data entry is complete then click on export data and check the export file in the folder C: /DISE OUTPUT . Please copy Deptdata.mdb file from DISE OUTPUT folder and copy into CD.
- Relevant Supporting documents (signed and stamped by Head of the Department) of the employees those are marked during data entries as Disable /Pregnancy /Lactating woman /On Long leave/BLO/medical certificate of very Chronic Patients etc should also be submitted along with Undertaking and checklist .
- Data of drivers/security guards should also not be entered in DISE Capsule.
- List of Class 4 employees / Drivers should be enter in prescribe performa (Excel Format ) submit this excel file in the CD along with DISE Capsule data.
- Please download & install the DISE capsule and then download the DeptData.mdb file and Dise capsule will ask for the mdb file when run first time select the Deptdata.mdb file .
- Old data of your office/department is already provided in the DeptData.mdb file please edit/add/ update it through DISE capsule software
- If old data of your office is not available in your DeptData.mdb file then enter freshly all your employees data in DISE software.
